Case Manager Duties

Duties & Responsibilities
  • Complete an intake assessment to identify  strengths and needs of the child and family.
  • Develop a service plan that outlines needs, goals, and services.
  • Help determine how to access services/supports if insurance does not cover.
  • Help make referrals for services/supports.
  • Help complete Medical Assistance applications.
  • Write safety plans and assist with crisis intervention.
  • Meet with your child/family at least once per month to monitor progress and address concerns.
  • Provide information about community supports/activities for your child/family.
  • Communicate with teachers, therapists, and others to assess needs and monitor progress.
