1. How do I apply for medical assistance?

For online to the MNsure website to apply for Medical Assistance. However, if you are 65 or over, certified disabled through Social Security Administration, or are receiving Medicare, please contact us at 952-445-7751 or toll free at 1-877-445-7750 or print a Health Care Application for Certain Populations (PDF) and mail to or drop it off at our office.

2. What are my options for transportation if I'm open on Medical Assistance?

Contact SmartLink Transit at 952-496-8341. However, if you are open to a health plan on Medical Assistance, you will want to call the phone number on the back of your medical card or the phone number below, depending on what plan you are on.

3. What is the policy for reporting changes and how do I do it?

Most changes need to be reported to your worker for cash and health care programs within 10 days of the change. You can report changes in writing, by phone, by email or by filling out and submitting a Change Report Form (PDF) to the agency.

4. How do I find out the status of my application?

Contact your worker directly; if you are not sure who your worker is, contact the agency at 952-496-8686.

5. My family member is in a facility. What do I need to do to get help?

It is best to contact the agency in this situation. There are many things to take into consideration. For example, the type of facility he/she is looking to go into or if the client has a spouse that is going to remain in the community. We want to be sure the client gets all the necessary forms and gets referred to social services, if necessary, to receive the benefits they need.

You cannot apply for Long Term Care Assistance more than one month in advance of when the person needs the coverage to start. In most cases you will need to use the Minnesota Health Care Program Application for Long Term Care Services. However, if you are already on assistance contact your worker to obtain the correct forms.
